Physicians will combine five specialties plus accounting.
In the Komi Republic, in order to close the shortage of doctors’ personnel and give them the opportunity to earn up to 200 thousand rubles a month, part
He ordered to find a hundred district police officers. For a whole year they will study remotely, take exams, receive certificates, and in a year they will be transferred to another level.
The selected doctors will acquire additional specializations that are lacking in the region: cardiologist, neurologist, surgeon, obstetrician-gynecologist and pediatrician. The pilot project will be carried out in 2022-2023.
Golden mountains …
According to the plan, 2 thousand people will be assigned to one doctor of the new formation in rural areas and small towns.
To fulfill the state assignment, doctors-entrepreneurs will receive money through the compulsory medical insurance scheme.
The budget for such a site can be 8 million rubles per year. According to officials, with these funds, the doctor will be able to hire two paramedics for repeated appointments in the office and at home, as well as a nurse for diagnostics.
For patients, these services will remain free of charge.
At the same time, the load on the doctor drops several times, and he takes over five more specialties.
At the same time, it is assumed that the salary of a doctor will rise to 200 thousand rubles, paramedics will be paid up to 100 thousand, and a nurse — 50-80 thousand rubles.
True, such a doctor will have to deal with all the accounting himself, but he will also be taught this.
… and the harsh reality
Experts have already pointed out the disadvantages that may arise with such a system.
So, the co-chairman of the All-Russian Union of Public Associations of Patients, ONF expert Yan Vlasov noted that to conduct a private practice, a whole package of documents is needed, including a license for the premises in which the appointment is held.
And many offices of the current polyclinics, in view of their deplorable state, would hardly be able to obtain such permits.
Also, Vlasov believes that private offices are more vulnerable in cases of litigation, and the question of doctors’ salaries caused bewilderment among the specialist, since according to the compulsory medical insurance tariffs, an appointment with a general practitioner costs about 200 rubles.
Meanwhile, Lyubov Khrapylina, Professor of the Department of Labor and Social Policy of the Institute of Civil Service and Management, RANEPA, urged the Governor to recall the Constitution, which spelled out the obligation to provide people with free and high-quality medical care.
She also urged not to shift the question of earnings from their shoulders to others.
On condition of anonymity, the doctor who worked as a district pediatrician in the late 1980s spoke with
According to her, doctors actually treated about 1,600 children, working for two doctors. For this they were paid only half.
For example, at a normal rate of seven calls, the doctor could serve 25, but they no longer paid for it.
Waiting for answers
The founder of OMS in Russia, Honored Economist of the Russian Federation Vladimir Grishin in an interview with
According to Grishin, it is pointless to evaluate the project until the authorities answer these questions.
Of course, such an experiment can be carried out, but only taking into account all the constituent parts of this process, since this is a lot of money. Experiment can prompt the transition to a new model.
In it, the state does not support doctors, but gives them such rights themselves.
At the same time, the expert noted that if doctors switch to free bread, the system of the Ministry of Health will be able to save about 30% of funds, «
At the same time, Grishin explained that the very idea of business doctors was taken from abroad, but in other countries, such individual entrepreneurs are allocated a budget commensurate with the number of assigned patients, and then these funds are used to organize the work.
